Varicocele Treatment: What You Need to Know About Options, Recovery, and Fertility

Varicocele is one of the most common and correctable causes of male infertility in India, affecting roughly 15% of adult men and up to 40% of men being evaluated for fertility concerns.

𝗪𝗵𝗮𝘁 𝗜𝘀 𝗮 𝗩𝗮𝗿𝗶𝗰𝗼𝗰𝗲𝗹𝗲?
A varicocele is an enlargement of the veins within the scrotum. These enlarged veins can raise the temperature inside the scrotum, which in turn affects sperm production and quality. Over time, this can lead to a lower sperm count, poor sperm motility, quality and reduced fertility.
Many men with varicocele have no symptoms at all and only discover the condition during a routine fertility evaluation. Early detection can make a significant difference in treatment outcomes.

Not all varicoceles cause symptoms. When they do, common signs include:
▸ A dull ache or dragging sensation in one or both testicles, especially after prolonged standing or physical activity ▸ Visible or palpable enlarged veins in the scrotum (sometimes described as a 'bag of worms') ▸ Unexplained difficulty conceiving after 12 months of trying

The connection between varicocele and male infertility is well-established. Elevated scrotal temperature caused by the pooling of blood in dilated veins interferes with sperm production. This can result in:
▸ Reduced total sperm count ▸ Poor sperm morphology (abnormal shape) ▸ Decreased sperm motility (movement) ▸ Elevated oxidative stress markers in semen
The good news is that varicocele treatment has been shown in multiple studies to improve sperm parameters in a significant proportion of men, often improving the chances of natural conception or the success rate of assisted reproductive techniques like IUI and IVF.

For men with no symptoms and no fertility concerns, a 'watch and wait' approach may be appropriate. Regular monitoring by a urologist or andrologist is recommended.
𝟮. 𝗩𝗮𝗿𝗶𝗰𝗼𝗰𝗲𝗹𝗲𝗰𝘁𝗼𝗺𝘆 (𝘀𝘂𝗿𝗴𝗶𝗰𝗮𝗹 𝗰𝗼𝗿𝗿𝗲𝗰𝘁𝗶𝗼𝗻)
The most common and well-studied varicocele treatment is varicocelectomy, which involves surgical ligation (tying off) of the abnormal veins. This can be performed using the following approach:

𝗥𝗲𝗰𝗼𝘃𝗲𝗿𝘆 𝗔𝗳𝘁𝗲𝗿 𝗩𝗮𝗿𝗶𝗰𝗼𝗰𝗲𝗹𝗲 𝗧𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁
Recovery time depends on the procedure chosen:
▸ 𝗠𝗶𝗰𝗿𝗼𝘀𝘂𝗿𝗴𝗶𝗰𝗮𝗹 𝘃𝗮𝗿𝗶𝗰𝗼𝗰𝗲𝗹𝗲𝗰𝘁𝗼𝗺𝘆: Most patients are discharged from hospital within 24 hrs, return to desk work within 3–5 days and resume full physical activity within 2–3 weeks
Sperm parameters typically begin to improve within 𝟯 𝘁𝗼 𝟲 𝗺𝗼𝗻𝘁𝗵𝘀 following varicocele treatment, as this is the time required for a complete sperm production cycle.
